PR Performance Lab - Photo Release Form

 

I understand that during the course of my participation in coaching, personal training, weightlifting and other physical training at PR PERFORMANCE LAB, LLC (henceforth referred to as PR PERFORMANCE), photographs may be taken of me, and used by PR PERFORMANCE for advertising PR PERFORMANCE either through written, print, internet, social media and/or otherwise advertising. My signature below states my agreement to allow my photograph to be taken by PR PERFORMANCE, its employees and agents, and used at their discretion. Further I hereby agree to the following Waiver and Release of Liability:

 

1. Waiver and release of liability:

   1. I hereby release and hold harmless PR PERFORMANCE its officers, directors, employees, agents, volunteers, and contractors (collectively, “Releasees”) from any claim, demand, loss, liability, damages, and attorney fees and costs whatsoever arising from, related to, the use of my photograph when the photograph is used for advertisement of PR PERFORMANCEin written, print, internet, social media and/or otherwise. 

   2. As between each of the Releasees and me, as it relates to my photograph being used as described above, I will not sue them for any injury, perceived or real, occurring to me as it relates to the use of my photograph for PR PERFORMANCE advertisement purposes as stated herein. If I do file a lawsuit against them or use them to engage in any action, litigation, arbitration, or otherwise, and they incur attorney fees or other fees/costs of any kind, I will indemnify and reimburse PR PERFORMANCE or any of the Releases from liability and the costs or damages PR PERFORMANCE or its Releases incur as a result of my actions. 

   3. This agreement shall be binding on my estate, heirs, executors, administrators, successors, and assigns, as well as any other party asserting a Claim on my behalf or on behalf of my estate.
